Fees:​

• Evaluation Fee (taxes included): New patients & 1 year renewals are $160, renewals for 2 years is $270.

• State Medical Cannabis Card Fee: $38.50 (new patients & 1-year renewals, $77 for 2-year renewals)- Separate from Evaluation cost.

•  Compassionate pricing available for patients on SSDI or SSI with valid proof.
